'The Churches of Liverpool' by David Lewis - index

This book, describing many of Liverpool's Churches and chapels, is well illustrated, but lacks an index. A few years ago a kind member of L&SWLancs FHS sent me a copy of one she had made, which I find very useful.
If forum members want one, let me know.
I am sorry that I have mislaid the member's name.

I'd love a copy if it's possible - I've never understood why it was done without one.
