Help! Can't find medal card

This query is for someone who came to the Crosby help Desk on Friday.

They had a medal which was their grandfather's. Around the side was 695622 DVR S. A. Smith R.A. which I took to mean his number and the fact he was a Driver in the Royal Artillery.

Can anyone see his medal card please?
